Request to Book Church Facilities
PLEASE READ THIS FIRST: The request form located at the link below is to be used by church members only who want to request the use of some part of the church facilities for a gathering. If you have multiple requests, please complete one form for each requested date unless your request is for a weekly or monthly event. In that case, please state those details in the "Additional Comments" field on the form.
Submitting this form does NOT confirm your reservation of the facilities. Once submitted, one of our staff members will get back to you within a couple of days to let you know if your request can be approved or not. If you have an urgent need, please contact Kevin Thomas at (615) 293-1923.
